Mostly due to lack of Dental Insurance that adequately covers above and beyond an annual $1000 cap to any major work. As a result many let their oral health decline further since they can ill afford to pay the $3000 to $10,000 required to repair what was damaged over many years. This is probably the most over looked medically related issue exacerbating the physical health of Americans.
